Improve formatting

When inputting forename(s) it would help if the first letter is capitalised.

There are controls for capitalisation in the Change Field Parameters screen, but I think the issue may be that those settings are not actually applied until after the record is submitted, so the request here is that settings for capitalisation, etc. all be applied immediately. At the moment it appears that the background colour takes effect immediately, but not capitalisation or font size.
