Add mandatory check box about personally identifiable information

Problem

We want to make sure that nobody accidentally uploads data that include personally identifiable information.

Solution

Let's add a schema option, in the first mandatory page I guess?, for a mandatory check box that says:

"I confirm that this dataset contains no personally identifiable information."
