When using Umbraco's bulk publishing features (such as Publish with descendants), only items with changes pending (i.e. items in draft mode) are actually published for performance reasons (source).
This is not intuitive, and is not explained at all by the current UI. To figure this out, an editor must be aware of the total number of descendant items they are attempting to publish, and then notice the discrepancy in the notification that appears after a bulk publish is completed.
Furthermore, in some circumstances it is desirable to have items without changes be published regardless. For example, when a property has been added to a document type, publishing ensures that that said property is added to the Examine indexes.
Other CMS' solve this by having different "types" of publish actions. Sitecore for example has the concept of a "smart publish" (only publish items with pending changes) and "republish" (publish everything).
A similar option could be added to the Umbraco Publish with descendants dialog to make this function available when necessary, and disambiguate what the feature does by default.
