Adapt interfaces document to match current reference implementation #51

The reference implementation is based on the https://w3c.github.io/voiceinteraction/voice%20interaction%20drafts/paInterfaces/paInterfaces.htm document. However, some things were adapted to make designed APIs work or covered parts that were not yet described.

Update the interfaces document to match the current reference implementation.
