For the spec, this feels a bit odd, but since it is so generic, it works for me.

Yes, I agree it is a little bit weird. I suppose to keep it so we can report things like typo, spec confusion, algorithm error, incorrect description, etc.

Rationale for this change
As we have migrated from JIRA to GitHub issues, it would be good to provide issue template similar to parquet-java repo.
What changes are included in this PR?
Add GitHub issue template and modify PR template.
Do these changes have PoC implementations?
Not a format change.
