crypt: bundle md4 and use it with mbedTLS

mbedTLS v2 optionally includes MD4 support, but it needs to be
explicitly opted-in during build time. mbedTLS v3 removes it entirely.

Provide our own md4, using the reference implementation from RFC 1320.
Use it when compiling with mbedTLS (at any version level).
