Move the template to .github/ISSUE_TEMPLATE/ in preparation for turning it into an issue form that also works on the mobile GitHub app. Since .github/ISSUE_TEMPLATE/ can contain multiple templates give it a unique name and the bare minimum of YAML front matter that GitHub requires for issue templates in that folder.
